non-discrimination disclaimers

Hello All,

We are preparing a new scholarship application and wanted to include a text stating that we are asking certain questions regarding race, ethnicity and gender for purposes of statistics only. We wanted to actually include a non-discrimination disclaimer text in the application.

Is anyone doing this?

Do you possibly have a text you might like to share?
