Hi there - We're interested in implementing a survey for our grantees of our LOI and Application processes.
I'm wondering if anyone here has used the https://grantadvisor.org/survey.php?ein=41-6038262 in their applications? If so, how did you word the question, how do you retrieve the results and what were your results?

Sorry, we haven't utilized this survey. However, we do regular feedback questions on our applications and our grant reports (basic stuff like how was our customer service, how many hours did it take you to complete the application, etc.). We keep these feedback questions to 3 max per form.

We use Grant Advisor and include it in applications. See attached screen shot of question wording. We also promote and invite reviews in our email communications, in organizational newsletters and and during webinars. The response rate was low initially but now we receive a fair amount consistently each year.
