Multi-fund signature page

I would like to ask if anybody has and/or is drafting fund agreements with a signature page to accommodate when donors would like to establish multiple funds? We recently had a donor that was interested in establishing a scholarship, field of interest (with multiple purposes,) and designated (with multiple purposes.)
